Every registered member receives a unique 10-digit Medical Identification Number. Your MIN is a membership reference that helps First Sayve Health connect your verified profile to the appointments and care pathway arranged for you.
When you attend an appointment coordinated through the program, the partner center may ask for your MIN to confirm your membership and locate the correct coordination information. It helps reduce confusion when different teams are involved in your care.
Your MIN is not a payment card, hospital record number, government identity document, or insurance policy number. A hospital may still create its own patient record and ask for the identification and consent documents required by its normal procedures.
Keep your MIN private and share it only with First Sayve Health or a partner center involved in an appointment we arranged. If you believe the number has been exposed or used incorrectly, contact our care team for guidance.
The MIN supports coordination, but it does not by itself confirm eligibility for a particular service or guarantee that treatment costs will be approved. Each care decision follows clinical assessment and program review.
